Insurance & Billing Information

Commercially Insured Clients:

In-Network Insurances: Moda, Providence, Kaiser NW, PacificSource Commercial, and Medicare*

  • ACC will bill any in-network insurance as a courtesy.

*As a clinic, ACC is in-network with Medicare, however, ACC’s medicare providers are currently full.

Medicaid & Oregon Health Plan:

ACC is currently contracted with Lane County’s Trillium Community Health Plan (OHP) and PacificSource Community Solutions* (OHP).

*PacificSource Community Solutions is no longer the CCO of Lane County; if you are a PS Community Solutions member, please contact us about your treatment options during this time of transition.

Private pay rates:

For any clients seeking to pay without the use of insurance, a private rate is available. All fees will be agreed upon prior to your initial session.

  • Licensed Rate: $165.00

  • QMHP/Registered Associate Rate: $125.00

A sliding scale is also available for work with ACC’s Mental Health Interns/QMHPs/Registered Associates:

  • QMHP/Registered Associate Sliding Scale: $110.00, $85.00

  • Mental Health Intern Sliding Scale: $50.00, $35.00

Late cancelation & No show fees:

  • The cost of session.

Changes in insurance or payment:

Clients are responsible for informing ACC about any changes in insurance, updates to credit cards, or other applicable insurance information that may impact billing.

Good Faith Estimate

In compliance with the No Surprises Act, you’re entitled to a Good Faith Estimate explaining how much your medical care with cost. If you are not using insurance, you have the right to receive an estimate of how much your medical care will cost (including medical items and services). You have the right to receive a Good Faith Estimate for the total expected cost of non-emergency items or services including medical test, prescription drugs, equipment, and hospital fees.

You can ask your health provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is a lease $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure you keep a copy of your Good Faith Estimate for your records.
